전 세계적으로 고강도 항레트로바이러스 요법이 시작되면서 기회 감염의 빈도가 과거보다 감소되었으나 국내에서는 면역 저하가 진행된 상태에서 첫 진단을 받는 환자들이많아 진단 시에 기회 감염이 동반된 환자들이 많다. 우리나라에서 균혈증은 여전히 AIDS 환자 사망의 6-10%의 원인을차지하는 중요한 감염이다. 하나 이상의 균에 의한 균혈증이14.5%에서 발생할 수 있는 것으로 알려져 있으나, MAC, S.
pneumoniae가 동반된 균혈증은 매우 드물고 아직까지 국내에서는 보고된 바가 없다. 본 증례는 진행된 AIDS 환자에서혈액 배양에서 MAC, S. pneumoniae가 확인된 경우로 MAC 는 일반 배양 검사로는 동정이 어렵고 항산균 배양 검사를통해 동정할 수 있기 때문에 CD4+ T세포수가 낮은 HIV 환자에서 적절한 항생제를 사용했는데도 발열이 지속될 경우일반 혈액 배양 검사뿐만 아니라 항산균에 대한 혈액 배양검사를 같이 시행하는 것이 좋겠다.
