In cancer patients, transfusion with blood components immediately before, during or after major surgery is common. The most common causes of red blood cell (RBC) administration blood loss and hemodilution and transfusion-related immune modulation is well-known adverse effect following transfusion of blood product. An impaired immune surveillance system as a consequence of the administration of substance in packed RBCs (residual viable leucocytes, various biological response modifiers present, and apoptotic cell) has been linked to cancer recurrence and progression. Of importance, the cancer cells can complete the metastatic cascade during the initial few days to weeks after surgery. Thus, the perioperative period is a critical time window for cancer recurrence, and it is of cardinal importance to reduce the exposure of allogeneic blood and to develop preemptive approaches to reduce the immunomodulatory effects of RBC transfusion for recipients who are already vulnerable to metastasis due to surgery-induced stress, inflammation, and immunosuppression. This lecture attempts to provide an overview of anesthetic strategies to reduce the negative impact of allogeneic RBC transfusion on postoperative cancer metastasis and recurrence based on research outcomes from our center.
