Background and Objectives: Many reports have advocated the feasibility of using an endoscope for the treatment of cerebrospinal fluid(CSF) rhinorrhea. Previous reports about efficacy of this endoscopic approach in Korea have limitation that contained too much iatrogenic cases. Aim of this report is to determined the effectiveness of endoscopic repair of CSF rhinorrhea due to variety of causes and analysis of differences by etiology.
Materials and Method: Fifteen cases of CSF rhinorrhea were reviewed retrospectively.
Results: Eighteen leaks occurred in 15 patients. Five of the 15 had iatrogenic injury resulting in CSF rhinorrhea, 7 had leak from head trauma, and 3 developed spontaneous CSF leaks. There were 3 cases of meningitis and 3 cases of pneumocephalus among 15 cases. These tended to be more common in patient with head trauma andspontaneous CSF rhinorrhea, compared to iatrogenic. Transnasal endoscopic repair have been applied to all 15 cases. Leak sites were repaired with nasal mucosal graft, temporalis muscle fascia, bone chip. Fibrinogen-based collagen fleece(TachoComb®) was used for 2 cases which have large skull base defect. Thirteen cases out of 15 repaired successfully after first surgery. Two patients of CSF rhinorrhea associated with accidental head trauma required revision surgery.
Conclusion: Our result showed that endoscopic repair of CSF rhinorrhea is an effective treatment, regardless of etiology. However, accidental head trauma patients had a higher rate of complication such as meningitis or pneumocephalus, multiple defect, and surgical failure compared to iatrogenic cases.
